Population health vendor CareSkore raises $4.3 million – with the help of NFL QB Joe Montana

CareSkore tech uses predictive analytics with the goal of improving clinical and financial outcomes through personalized, real-time interventions in diverse patient populations.

CareSkore, a population health management technology vendor, has raised $4.3 million in its initial round of funding. And former San Francisco 49ers quarterback Joe Montana is part of the team of investors backing the upstart.

CareSkore technology uses predictive analytics with the goal of improving clinical and financial outcomes through personalized, real-time interventions in diverse patient populations.

In addition to Liquid 2 Ventures, which is run by Montana, other investors include Cota Capital, which backed Google, Uber and DropBox; Storm Ventures, which backed Marketo and other mobile and security companies; and Rising Tide Fund, which backed Oracle, Palantir and Cloudera. 

CareSkore’s two co-founders are chief medical officer Puneet Dhillon Grewal, MD, an internal medicine physician and cardiologist, and CEO Jaspinder Grewal, a computer engineer with an MBA from the University of Chicago Booth School of Business.

“CareSkore helps measure the clinical and financial risk of hospital patients based on their electronic medical record and claims data,” the company said. “This allows hospitals to focus on patients who need the most care and management, both in hospital and after discharge. The company was launched in 2014 as a direct response to the Affordable Care Act, which holds hospitals and providers accountable for the quality of care they provide.”

The company conducts predictions in real time, based on a combination of clinical, labs, demographic and behavioral data. It creates individualized, disease-specific workflows and care plans that enable physicians, nurses and care teams to detect and monitor patients, and in turn focus on patients, CareSkore said.
