Fort Lauderdale, Florida-based Broward Health has tapped Cerner for its HealtheIntent population health platform, the company announced.

The HealtheIntent platform analyzes patient data from its EHR, claims processing, laboratory systems and more to offer a real-time snapshot of the care and health trends across its patient population.

Broward, a public health system, will also use Cerner's HealtheLife: Engagement platform, which takes the data and uses it to suggest tailored patient engagement steps, such as suggestions for wellness tools and other health tracking options.

"Our providers will have information to more easily identify at-risk individuals and apply prevention methods to fulfill our strategy of improving the health of our community and quickly intervening for high-risk patients," said Mark Sprada, corporate chief nursing officer at Broward Health, in a statement.

In addition to Cerner's population health and patient engagement platforms, Broward will also leverage Cerner's Readmissions Prevention software, which uses patient data to predict which ones may be at risk for readmissions, an especially important platform as health systems face penalties from the Centers for Medicare and Medicaid Services over high readmission rates.

In fact, four Broward hospitals will see their Medicare payments cut between 0.25 and 1.1 percent in 2016 in the program, according to CMS data.

"With market pressures from industry changes such as new payment policies, reduced spending growth and the transfer of risk to providers, there is an increased need for providers to focus on the overall management of health in its community," Cerner Senior Vice President of Population Health Matthew Swindells said in the announcement.

Broward, which already uses Cerner as its EHR supplier, is the latest system to grab Cerner's population health platform. In October, Geisinger Health System said it would install Cerner's platform, though it currently uses Epic for its EHR solution.

Monetary terms of the Broward-Cerner deal were not announced.
